You can mail or fax your order to us. Download and print the order form. If faxing the order, include credit card information. Mailing in the order, include Visa, MasterCard, Discover, American Express credit card information. USA customers may also pay by personal check* or Money Order. We also accept Canadian postal money orders in US dollars.

*Personal checks may delay the order to allow time to clear.
